Getting Started With Dental Designs Is Easy.
Dental Designs of Washington tries its level best to put technology to use on your behalf, not only within the dentistry itself, but as part of our on-boarding and communication process. But technology is only as helpful as the people behind it all. That's why you'll meet with a member of our staff on your very first visit to discuss your concerns in person.
But first? There's a little paperwork that needs to be completed for us to be able to welcome you to our practice.
Complete Your Adult Or Minor Medical And Dental History Forms.
This is an important first step once you’ve decided to become a patient. Your secure and private medical and dental record is where we start and build our relationship. It is generally easier to complete these forms at home, where you have access to personal records and information.
If there are sections of the history you can't complete? Don't worry — we'll work through them together in the office. Please just remember to bring your completed forms with you!
To use the on-line interface, you will need to have an appointment date scheduled with us. Please use our simple on-line interface to securely complete your adult or minor patient information forms. (This spares you some writing by hand — and by doing them at home, you have access to the information you'll need.)

If you would rather write them out by hand or don't yet have an appointment date, download and print the forms from the link below — we do urge you to complete them before your first visit so we can get started immediately.
REMINDER: If you have past dental records and X-rays, be certain to obtain a copy of them prior to your visit, and bring them to your first appointment!
Remember Your Insurance Documentation. And Know Our Payment Policies.
Even though you'll provide information about your individual or family insurance plan on your patient forms, remember to bring your current insurance card with you. We accept most major dental insurance plans including United Concordia, Delta, Guardian, Cigna, Aetna, MetLife, Coventry, UPMC, Gateway and United Healthcare.
We also work with some fixed benefit dental plans, and also accept cash and most major credit cards. Should you need financing, we utilize nationally available Care Credit as a potential third-party finance partner — and can check on availability right from our office.
REMINDER: Payment for services and insurance copays are due the day of service, minus any adjustment for insurance. We will make every effort to accommodate your unique needs!
Your First Visit To Dental Designs Is Like...
...starting something new, or something that's good for you. Like going to the gym, or choosing the salad bar instead of the buffet. The important thing to remember is that everyone in the room, and in the practice, is here to help you feel comfortable and to improve your dental health.
First, one of our administrators will verify your history and insurance documentation and get you acquainted with our practice.
Then you'll meet with one of our hygienists or doctors, who will talk with you (or talk with parents if treating a minor child) about your medical history and current dental concerns. Then we will perform a complete clinical examination.
If treatment is indicated, we will begin our own diagnostics like panoramic or intraoral digital X-rays.
Your dentist will discuss the findings with you and present treatment recommendations specific to your needs. You should expect your first appointment to last approximately 1 hour; and perhaps a little longer depending on your unique circumstances.
